Demodex Blepharitis
What it is
Demodex blepharitis is a form of eyelid inflammation associated with an overgrowth of Demodex mites on the eyelashes and eyelid margins. A key clinical sign is the presence of collarettes, which are cylindrical waxy cuffs at the base of the lashes.
Common signs and symptoms
- Itching along the eyelid margins
- Red, irritated eyelids
- Crusting or debris at the base of the lashes
- Dry eye symptoms
- Ongoing blepharitis that keeps returning
How it is diagnosed
Demodex blepharitis is often identified during an eye exam by looking for collarettes and other signs of eyelid inflammation. Current expert guidance supports treating symptomatic blepharitis when collarettes are present.
Treatment options
Treatment may include:
- in-office eyelid cleaning such as lid exfoliation
- home eyelid hygiene
- prescription treatment when appropriate
The only FDA-approved prescription medication specifically indicated for Demodex blepharitis is XDEMVY, which is used twice daily for 6 weeks.

If Demodex blepharitis is not addressed, it can continue to contribute to chronic irritation, lid inflammation, and dry eye symptoms.
